CCD Recommendations

  1. Transition planning and services should be evidence-based, and must include:

  • School-based preparatory experiences,

  • Career preparation and work-based learning experiences,

  • Youth development and leadership, connecting activities such as services, activities,

  • Supports that help youth gain access to chosen-post-school options, and

  • Family involvement and supports.
