Requirements for CEU Credit

To earn NA CEU, you must meet ALL the following requirements:

1. Participate in the entire event: 1.5 hours on September 6, 2012.
2. By CEU DEADLINE: September 16, 2012
   Download, complete and submit CEU Request Form to TACE Region IV.

CEU Request Form

(Note: After Account login, available after event and until CEU deadline.)

1. Login to MyTACE or create New Account
   Note: Each person who seeks CEU credit must have an Account; this same Account is used to to register for available events and request applicable education credit. For questions, contact TACE Region IV .
2. After successful login, from Account Manager, select event that you want CEU credit.
3. Select link: CEU Request Form
4. Download and complete CEU Request Form:
   • Participant, Address, Phone, & Contact
     Note: Except for SU ID Number & SSN (leave both blank).
   • Course Information (as provided per instructions)
   • Sign and date Participant Acknowledgement
5. By CEU DEADLINE: September 16, 2012
   Submit completed CEU Request Form to TACE Region IV via Fax: (404) 541-9002.
6. Upon receipt, TACE Region IV will verify your CEU Request.
7. Your CEU Request Form will be sent for processing by University College at Syracuse University.
